How to Apply

For most CMSE programs, all you need to do is fill out an application form and send it in with the
check for the registration fee. If you have a printer, you can print the application form. Application
forms are also printed in our newsletter Chimes, or you can obtain one from us by any of the following
methods:

Call CMSE toll-free at (800) 428-1045. Ask for our Program Manager Sherry Coble, or leave
a message for her, with your name and address, requesting a CMSE application form.
Fax your request to us at (919) 962-0588, including your name and address. If you wish, we
will fax a copy of the form back to you.
EMAIL us at scoble@email.unc.edu. Be sure to include your name and address.

If you wish to apply for more than one program, please note that each program requires a separate
application form and a separate registration fee. Also, each person applying must fill out a separate
application form (you can duplicate as many copies as you need).

Be sure to fill in all the information requested on the form. Program directors make use of this
information in deciding whom to admit, so missing information is likely to harm your chances for
admission.

Registration Fee

Most CMSE programs require payment of a $25 registration fee (exceptions are noted on the
program information pages). If your school or school system is paying your fee, include a note to that
effect with your application and let us know whether we can expect the fee automatically or whether
we need to invoice someone at your school or your central office. In the latter case, provide a
complete address for the invoice.

When we receive your check, we are required to deposit it immediately (state agencies are not
allowed to hold checks for future deposit). Of course we will refund the registration fee if the program
is full or your application is not accepted. If you are placed on a waiting list, we usually keep the fee
until a decision is made on your application, but we will refund it if you wish.

Summer Program Admissions

There is usually a common deadline date for our summer programs (for 2001 the date is March 10).
Early applications will be considered first, all other things being equal, so apply as early as you can. No
admission decisions are made until the deadline date, so expect to receive word of admission in late
March. After the deadline, if space remains in the program, new applications will be considered within
a week or two of being received. Check CMSE Online for the information on the status of each
program!

If you are accepted for a summer program and find later on that you will not be able to attend, the fee
is refundable if you notify us on or before May 31. Otherwise, fees are not refundable once you have
been accepted into an activity.

License Renewal Credit

Nearly all CMSE workshops and institutes provide North Carolina license renewal credit; the amount
of credit is shown on the program's information page. However, only your school system can approve
your use of this credit in your professional development plan, so be sure to consult your staff
development representative to make sure the activity will be accepted.

When you complete an activity successfully, we will award you a completion certificate endorsed by
the UNC Mathematics and Science Education Network. Take this certificate to your staff
development representative. Be sure to keep a copy for your records!

Graduate Credit

Graduate credit is only available for a few CMSE programs; if it's available, there will be a notice of it
on the program's information page. In order to receive the graduate credit, you must pay the
University's continuing education fee. In addition, there will be additional course requirements set by
the program's instructor.

Stipend Payments

Many summer activities provide stipends for participants. Funds for these stipends are provided by
external grants. By state regulation, we can't submit requests for stipend checks until after the activity
ends; we have to provide the attendance records of the activity for documentation. The University
sometimes requires several weeks to approve, process, and print checks we request, so please allow
at least one month for the check to arrive. If you haven't received it one month after the activity ends,
call us at (800) 428-1045 or send us an email message at scoble@email.unc.edu.
