Essential Lifestyle Planning
Focused on Developmental Disabilities


Essential Lifestyle Planning has been a form of Person-Centered Planning in North Carolina. This training prepares the learner to facilitate and develop an Essential Lifestyle Plan (a recognized best practice in person-centered planning). Participants will have a practical 'hands on' experience of working with a focus person in the development of their plan. Additional exercises include making community connections, action planning, and writing person-centered goals that Medicaid will pay for and how to crosswalk content to the new state plan format and CAP-MR/DD plan format.

Person-Centered Thinking is a pre-requisite for Essential Lifestyle Planning - Facilitator and must be completed within 30 days prior to attending Essential Lifestyle Planning training.

Target Audience

  • Human Service professionals and paraprofessionals whose focus is primarily developmental disabilities
