This training event provides an overview of the Medicaid Service Definition, Targeted Case Management (TCM). TCM is a community support service that involves locating, obtaining, coordinating, and monitoring social, habilitative, and medical services as well as other services and supports related to maintaining the person's health, safety, and well-being.

Target Audience:

This presentation is designed specifically for case managers and support brokers, and other professionals and paraprofessionals involved with someone who may be involved with this service whose focus is primarily developmental disabilities.
