Application

Dear Applicant:

The Upward Bound Program at The University of North Carolina at Chapel Hill is a federally funded educational program which prepares high school students for post secondary education. If you are interested in preparing for college, download and complete the application. 

The following instructions must be followed to complete the application process:

  1. Complete the entire application (parent, pages 2-7 and counselor/teachers, pages 8-10) by supplying the proper data requested by the preferred date, September 15, 2008.
  2. Submit a copy of parents’ 1040 or 1040A Income Tax Return for the previous year. Tax returns must have the signature of parents/guardians.
  3. Submit a copy of your high school transcript and standardized test scores.
  4. Sign a Release of Information Form so that your schools guidance department will release a copy of your grades and test scores to the Upward Bound Program.
  5. Submit three (3) letters of recommendation: two (2) from academic teachers and one (1) from your high school counselor and a 250-300-word essay.
  6. Return the completed application to the Upward Bound High School Coordinator at your school.

Ultimately, Upward Bound is designed to generate in program participants skills and motivation necessary to complete a program of study that leads to a college degree.

Student must be 9th or 10th grade at the time of admission to the program.

PRIVACY ACT STATEMENT:
The personal information that you give to the Upward Bound Director is sent to the federal government (Department of Education). The information is protected by the Privacy Act. No one may see the information unless they work with or for the Upward Bound Program or are specifically authorized to see the information. The information is necessary to determine if you are eligible to participate in the program and helps the government to measure your success. The Department of Education has the authority to gather information to help make Upward Bound a better program (20 USC 1231a).
