Video Authorization Form (DOC)
This form should be used in any ENGL courses where students
will be videoed for instructional purposes. Students must
be at least 18 years
old, must understand that they will not have the
opportunity to rescind the contract (e.g., the problem might arise
if a student becomes a celebrity), and must each sign individual contracts
(everyone in the class signing the same contract will not suffice).
