| As you know
hiring departments are required to send an EOE data form and
acknowledgement letter to each applicant for all UNC-Chapel Hill
EPA positions. |

| To assist
you in complying with this requirement, the Equal Opportunity
Office has created an online process for sending and
collecting this information from applicants. Applicants may
access the on-line form at

| Your
department or search committee should inform applicants of the
options to: |
| 1 - submit
using the online data form |
| or |
| 2 - download
the manual form and return the completed form by e-mail or mail |
| |

Hiring departments should provide the EOO
Recruitment Plan number in your e-mail or letter to
applicants along with the department name and position
title since applicants often apply for multiple positions.
The EOO Recruitment Plan number is required when completing the
EOE Data Form. |
