Chris is a 21yo male with mild mental retardation and Bipolar D/O currently living in a supported living apartment setting with a room-mate and 24 hour staffing.
Goal :
Chris will demonstrate the ability to identify, select and successfully
participate in (2) new self-determined recreation activities in order to
develop a meaningful daily routine that decreases his reliance on paid
providers, increases his involvement with non-disabled peers and
decreases his social isolation by (date ).
Objectives:
1) Chris will be able to identify a minimum of (3) recreation activities
he would enjoy participating in with peers.
2) Chris will contact by phone and a site visit a minimum of (3) specific community resources that match his interests and personal strengths.
3) Chris will be able to identify a minimum of (4) acceptable topics of conversation and verbally interact with non-disabled peers in an assertive manner during 75% of his social encounters.
4) Chris will be able to set up (organize and plan ) a daily calendar of self-determined recreation and education activities he would like to particiapte in with non-disabled peers for (3) months.
