Recreation Therapy Services
Possible Scope of Services

Recreation therapy assessments for development of person centered service plans.
New clients will receive a comprehensive RT assessment. TRS will establish a quarterly assessment schedule based on service plan review dates. TRS will conduct annual in-service training for case managers on developing meaningful and measurable recreation and leisure and social inclusion goals.
 
Individualized recreation therapy services designed to meet the treatment and habilitation needs of clients. Interventions include skill development in the areas of communication skills, relationships, self-esteem, problem-solving, decision-making, and community living skills. The TRS will link 1:1 services with staff training so direct support staff can reinforce gians achieved through recreation therapy services.

Development and implementation of periodic, time limited rehabilitation and skill building groups. Group services must be noted on the individual service/habilitation plan and address specific treatment and habilitation goals. Groups will include self-esteem, relationships, problem-solving, communication, and decision-making skills.

Training for direct care staff (contract agency staff) who will be delivering direct or ancillary services to clients. Training will reinforce recreation therapy goals, community inclusion and quality home recreation and leisure opportunities. Annual trainings will be offered to each residential provider/vendor, with follow-up in small groups or 1:1 trainings offered at each client’s home residence. These trainings will be offered during day and evening hours, and weekends. Trainings are designed to enable staff to meet privileging requirements and to reinforce person centered plans and services. TRS will develop a community resource guide for each residence, maintain a community resource bulletin board and files, and distribute weekly community activity information to residential providers.

Case management services. Services are designed to meet the educational, vocational, residential, health, financial, social, and other non-treatment needs of the client. Services emphasis linkage and integration of multiple services, advocacy on behalf of the client, supportive counseling, preparation and revision of the service plan, and monitoring the provision of services.

Associated team duties (team meetings, on call, supervision meetings, staff training, billing, documentation, supervision of graduate student, travel) There are a number of team duties that are not directly reimbursable but are key requirements of quality service.

Collection and coordination of quality assurance data addressing community inclusion and day and socialization services to provide team with feedback regarding quality of care provided by team, contract providers, and the community.

Liaison and educational outreach to community providers, including Parks and Recreation, Community College/Comp Ed, Library, volunteer groups, etc. TRS will contact at least 3 community resources per month, to provide an overview of Thomas S services and discuss future involvement of clients in classes, volunteer roles, and/or activities. TRS will offer training to providers as needed and requested. Information will be incorporated into resource guide and shared at team meetings.
