Instructions for completing Holiday Premium Pay Form

(See also Holidays and Holiday Pay policies on the Human Resources Web site.)

Department Name: Name of Department for which employee works.

Date of Holiday Worked: Date of which Holiday worked falls. Indicate whether "Standard" or "Personal" holiday.

Account Number: The account to charged for the Holiday Premium Pay.

For the employee's Home Department: Fill out only the 4 digit function number.

For any department other than the employee's Home Department: Fill in the entire account number.

Rate of Pay: Rate of employee's pay at the time of the holiday worked.

Hours Worked:

NSDP: Number of hours actually worked on the holiday(s) that are not subject to Shift Differential Pay.

CSDP: Number of hours actually worked on the holiday(s) that are also subject to Shift Differential Pay.

For Payroll Use Only: Do not write in this space.

Approved by: The employee's supervisor signs this, certifying that the hours being reported were actually worked.
