Self-Identification Policy
Please take the following steps to help ensure that students with disabilities
requiring reasonable accommodations are identified as early as possible. Early
identification of these students will facilitate any steps that must be taken
to ensure access to course materials.
1. Self-identification Forms
There are three points of entry into the graduate and professional school programs.
They are:
At each of these points of entry a voluntary self-identification form should be included with letters of acceptance sent to students. Your program should already have copies of this form. If you do not, please contact Jim Kessler at the Department of Disability Services at 962-8300.
2. Accommodations Statement
The accommodations statement highlighted below (or a link to this statement) should be included in all promotional literature, print or web-based, for a distance education course/module or program. It should also be included on the syllabi for all distance education courses/modules.
If using a link to the statement, it is recommended that you link from the phrase "Accommodations for Participants with Disabilities".
