We know that people have different schedules, and we want you to be able to complete your course pack order in a way that is convenient for you.  Here is an option that we hope you find easy and helpful!

• Everyone will need to fill out the Order Form, but only those with copyrighted material in their pack will need to fill out the Copyright Clearance Worksheet. It is important to fill these out as completely as possible. Copyright clearance from some publishers can take weeks so having the correct information will avoid unnecessary delays.
• These Word documents are designed for you to be able to type and save information into them.  This method is perfect as you can begin your order, save your information, and come back and complete them at your convenience or make any necessary changes.
• Once you have completed the forms simply email them as an attachment to coursepackpub@store.unc.edu, or you can print them out and fax them to us at 919-962-3759.
• Chuck Sockell, the Course Pack Publishing Manager, will email you once the information is received.  If you have not heard from us after 2 business days, please contact us at 919-962-4708.
